Youth Case Management
We offer Youth Case Management (YCM) services in both the Missoula area and the Bitterroot Valley. Case management can be provided in addition to the clinical team, or can be provided on its own. Case management provides the family with education about resources and services available in the community so that families can make informed decisions about what will best meet their individual needs. Case management then assists with referrals and linkage to the family's chosen services and resources, and monitors to ensure that the services and resources are meeting the family's needs. YCM services also assist the family with coordination of information and services among all team members involved and follow up as necessary to ensure the care plan is being implemented appropriately. Case managers serve as advocates for families in holding providers accountable, including Full Circle clinical services, schools, and other outside agencies involved in a family's care. Case managers can also continue to serve children when they are in a group home, in acute care, or in shelter care, whereas family based services cannot. Our Case Management program strives to empower families to drive the treatment process and navigate different systems. It focuses on building natural supports with the end goal of formal supports stepping out when the family feels supported enough to manage their own care. Outpatient Therapy Outpatient therapy may consist of family therapy, individual therapy, and/or group therapy. Family therapy is most often done in the home with multiple members of the family. It often includes multiple members of the clinical team as well, such as the care coordinator or the support specialist in addition to the Family Therapist. There are times when we will provide family therapy without the client present. This is designed for the parent to work with a therapist on issues that are affecting their ability to parent the way they want to, but that may not be appropriate or productive to work on with the child present.On occasion, Full Circle may also provide individual therapy to a child to supplement the family work. |
